• ESSAY (up to 500 words) addressed to Dr. Edward Leitão Memorial Scholarship Fund Committee, describing your language and cultural connections to the Portuguese-American community, your goals, and how you envision making a difference once you finish medical school 

•  2 letters of recommendation (from  teachers/deans/academic advisors/rotation directors, etc) addressed to Dr. Edward Leitão Memorial Scholarship Fund Committee

• copy of transcripts 

• copy of MCAT scores, if applicable
